 | Parents of K, 3rd and 7th Grade students: We still need dental forms for your students. Please have your dentist fill out the form ASAP. If you prefer, your child may have a school dental exam this spring. Please contact the Health Office for any questions. |

| Physical Reminders: State Law requires all students in K and 6th grades to receive a physical exam. Please remember to submit these forms from your family doctor as soon as possible. Extra forms are available in the Health Office. |
| Please help us get your child ready for 7th Grade, State Law requires all children entering 7th Grade to be immunized for Hepatitis B and Varicella (chicken pox). Check with your family doctor to be sure your child is properly immunized. and be sure to send a copy to the Health Office. Please call if you are unsure. |
